Samuel Horton “Sam” Fields, 70, of Madison, Ohio, passed away April 18, 2022 at his home surrounded by his family. Sam was born August 24, 1951 in Kingsport, TN to the late William and Mildred (Winegar) Fields. After graduating from Collinwood High School, he worked 28 years as a Locomotive Engineer for Conrail. In his spare time, Sam enjoyed bowling, fishing, softball, Formula 1 Racing, and was active in Greyhound Rescue. Sam had many accomplishments as a bowler. He has three rings, two for his perfect (300) games on January 7, 2003 and November 2, 2004, as well as one for a 299 game on October 25, 2000. He is survived by his sister, Sheila Faye Hammonds (nee Fields) of Wetumpka, AL; daughters, Samantha Marie Fields (Andre Yi) of Los Angeles, CA and Christine Kate (Michael) Watson (nee Fields) of Madison, OH; grandchildren, Sabrina Lynn Shoff, Joseph Paul Shoff, and Benjamin Mathew Paul Watson all of Madison, OH. A Celebration of Life service will be held from 4-6pm on Friday, April 22, 2022 at Lake Erie Cremation and Funeral Services, 1500 Harpersfield Rd, Geneva, Ohio 44041 (440) 361-2273.
