Lake Erie Cremation & Funeral Services requires that this Authorization Form be completed and signed prior to your pets 

cremation. We are pleased to answer any questions you may have regarding the cremation process for your Loving Pet.


THIS IS AN AUTHORIZATION TO PERFORM THE CREMATION PROCESS, A CONTRACT FOR  SERVICES RENDERED, AND A RELEASE FORM FOR TRANSFER OF CREMAINS CUSTODY

The Authorizing Agent authorizes the Crematory set forth below to carry out the cremation, and the directions and instructions of the Authorizing Agent contained in this Authorization Form.

permission to cremate  the pet (named above).

4. Release of Cremains
